MAXIM will provide services to eligible RECIPIENTS who sign MAXIMS consent form in accordance with the following rate schedule: Service Employee: rate- Influenza $30. & Pneumonia $45., Medicare Part B Participants: rate- Influenza No Copay*, and Pneumonia No Copay*,. Non-Medicare: Participants’ rate- Influenza $30. and Pneumonia $45.
